Credit Card Authorization Form
I hereby authorize EZ Loans Funding and/or its designated vendors to charge the credit card or checking account provided herein for the one-time charges as detailed and agreed upon in this document. This authorization applies solely to the charges that have been explicitly checked and agreed upon below. Any future or additional charges will necessitate a separate authorization form to be completed and signed.

***The following scenarios will require a separate invoice to be sent, do not use this section for appraisal payments on the property types: rural, multi-family, over 3500 sq ft, over 1M in value, commercial, land, construction***
Cardholder Acknowledgement:
I confirm that I am the legal cardholder or authorized user of the credit card or owner of the checking account listed above and that I have the authority to approve the specified charges.
I affirm that there is sufficient credit or funds in the account to cover the transaction amount.
I acknowledge that I have received, reviewed, and provided signed consent for the Loan Estimate, Truth-in-Lending Disclosure, and the Notice of Intent to Proceed, in accordance with federal regulations, prior to incurring any charges related to the Appraisal.
I understand and agree that all charges processed as per this authorization are non-refundable. This includes fees for credit reports, appraisals, verifications, and other services which are paid for in advance and rendered upon payment.
Consumer Rights:
I acknowledge my right to receive a copy of this authorization.
I understand my right to dispute any charges that were made without my permission or were inaccurately processed as outlined by the Fair Credit Billing Act (FCBA) and the Electronic Fund Transfer Act (EFTA).
Cancellation and Refund Policy:
Non-Refundable Charges:
All charges processed by EZ Loans Funding and/or its designated vendors are final and non-refundable. This policy is in recognition of the nature of the services provided, including but not limited to credit reports, appraisals, and verifications, which are paid for and subsequently performed immediately upon the receipt of payment. As these services are deemed completed once initiated, we are unable to offer refunds or accept cancellations once a transaction has been authorized and processed.

By submitting this authorization form, I acknowledge and agree to the non-refundable policy of EZ Loans Funding. I understand that once I authorize the charges detailed above and the services are rendered, I will not be eligible for a refund under any circumstances.
I understand that the entity that will appear on my credit card statement or bank statement will be "Red Tree Production".
